| BayPath Elder Services is now offering private care management. The program
is designed to provide easy access to eldercare on a private-pay basis for
those elders who are not financially eligible for state-supported care. The
private care managers have access to the resources and expertise of BayPath,
which includes information and referral, a large computerized database of
local, state, and national resources, registered nurses, health benefit counselors,
money management specialists and much more. Private care managers are seasoned
professionals who carry credentials in nursing or social work. All services
have the option of being billed on an hourly basis. To receive more information

For those living at home, private care managers can:
- Conduct in-home needs assessments in consultation with caregivers
- Research the best available care options that fit within the family's
resources
- Monitor care quality and report to family
- Assist with completing applications and claims forms
- Provide specialized consultation and training on caring for someone
with Alzheimer's disease or other dementia
- Serve as a liaison with health and social services providers
- Escort to medical appointments and report findings to the family
- Make referrals for legal, financial, or mental health needs
For those living in a facility, private care managers can:
- Consult with facility staff regarding a suitable care plan
- Monitor the quality of care through regular visits
- Report to the family on changes in a client's status or care issues
- Serve as a liaison with family, staff, client and his or her physician
- Assist with Medicaid applications and other paperwork
- Coordinate services upon discharge
- Consult with facility staff regarding the special issues of Alzheimer's
and other dementia patients.
