Elder Community Care

Improving Elders’ Access to Mental Health Services

 

Elder Community Care (ECC) is a non-emergent community-based program serving elders over the age of 60 living in BayPath’s 14 town service area who lack access to mental health and substance abuse assessment and treatment services. 

Elder Community Care strives to improve access to services, strengthen community networks, streamline communication, eliminate barriers to care, and decrease duplicated and fragmented services to community-based elders with substance abuse and mental health needs. 

 How do I know if I need  Elder Community Care Program Services?

Sometimes we need to talk to someone about our fears or our worries.  Sometimes we are experiencing grief and loss over a loved one.  At times, we may be over-using alcohol, drugs, or medications.  We may find ourselves in financial difficulties due to compulsive gambling.  Elder Community Care counselors are available to help.

Am I eligible to receive services?  

If you are over 60 living in the fourteen town service area, you are eligible to receive in-home assessment, counseling, and referrals for additional resources.  With your consent, a counselor will provide an initial home visit as soon as possible.

There is no charge for this initial home-visit.

 What happens during the home visit?

During the first home visit, a counselor will assess your needs.     A plan for additional visits will be discussed with you.  All decisions and services are with your approval and consent.

 

What about other resources?

Your counselor can discuss other community resources that may be helpful to you. Referrals are made with your consent.
