The nurses determine clinical eligibility for state funded programs, such as the Enhanced Services Program. The nurses create individualized care plans for BayPath elders who need help bathing and personal care. They will also assist in safety planning.

The nursing department determines eligibility for other programs such as Adult Day Health, Personal Emergency Response Systems, Choices Program, Home and Community Based Waiver, and Home Health Services for elders with MassHealth.

These programs help elders and MassHealth recipients remain independent in their community for as long as possible.

When all community alternatives have been explored and a higher level of care is needed, the nurses will determine medical eligibility for nursing home placement.
