BayPath Elder Services, Inc. offers home care and related services enabling older people to live independently and comfortably in their homes while promoting their well-being and dignity. Many BayPath services are free, others are based on one's ability to pay, and some are offered on a fee-for-service basis. |
| Founded in 1977 as a nonprofit corporation, BayPath has over twenty years experience serving and advocating for the needs of older citizens. As a designated Massachusetts Aging Services Access Point, we depend upon state and federal contracts as well as private donations to provide vital services such as care management, homemaking, home-delivered meals, health promotion activities, support to councils on aging and senior centers, legal help, senior employment, aid to the visually and hearing impaired, and mental health counseling. |

We act as a single point of entry for home care services. We guide elders and their families through the complex, long-term care system. Knowledgeable staff and skilled case managers ensure service quality and consistency for all clients. Upon request, we will:
BayPath offers a 24-hour On-Call System so that a case manager or supervisor may be reached after hours in the event of an emergency or an urgent situation that cannot wait until the next business day. When no other resource is available, an elder who is or is not a client, may apply to the BayPath Elder Care Fund. The fund was established in 1996 to make emergency grants, such as replacing household necessities when an elder's home is lost to fire or purchasing heating oil. Private donations support this fund. BayPath can meet the
needs of frail elders who require extended, consistent care through clustered
services. The Enhanced Community Options Program (ECOP) offers extensive
service packages to particularly vulnerable elders who are at risk for
nursing home placement.
Elder Housing Options In cooperation with the local housing authority, BayPath operates one congregate housing site located in Natick. Congregate housing is an arrangement of private apartments with shared living and activity space. The Housing coordinator arranges supportive services when needed, as well as plan educational and social activities which are important to the well-being of the residents. In conjunction with the Framingham Housing Authority, BayPath operates supportive housing at Memorial House in Framingham. Supportive Housing provides an assisted-living-like environment while maintaining an elders' independence. A Certified Personal Care Attendant is on the premises 24 hours a day, 7 days a week, to assist any tenant with an emergency. A Supportive Housing Coordinator is also on site daily. Memorial House has private apartments with shared living and activity space; supportive services such as housekeeping and personal care are also available. Recognizing the growing need for affordable housing options, BayPath co-sponsored with The Community Builders, Inc. of Boston, Mission Springs Elder Housing in Holliston. Built with six million dollars from the U.S. Department of Housing and Urban Development and some state funds, Mission Springs opened in the summer of 1997. The 75 units of supported housing are now fully occupied and BayPath provides an on-site coordinator and supportive services that may be needed. Coordination of Care BayPath's nursing department is available to members of our community to determine eligibility for other programs such as Adult Day Health, Personal Emergency Response Systems, Choices Program, Home and Community Based Waiver, and Home Health Services for elders with MassHealth. BayPath's registered nurses conduct pre-admission nursing home screening for Medicaid clients. The nurses also determine whether elders in our community are clinically eligible for home health services. Relief for Caregivers BayPath offers the Respite Program to alleviate physical and emotional strain felt by an overwhelmed caregiver. The service plan for respite may include social day care as well as home health aide, personal care, housekeeping, friendly visitors, and transportation Crisis Intervention Reports of elder abuse, financial exploitation,
neglect and self-neglect will be taken by Springwell (617-926-4100) and Elder Home Care
Services of Worcester area (508-756-1545). With the elder's consent and when necessary, these
agencies will intervene to protect the health, safety and legal rights of
an older person. The Crisis Intervention team often works with BayPath, and other community agencies, to assure
that these complex and sensitive situations are handled in confidence and
with compassion.
