Adult Family Care

The Adult Foster Care Program (AFC) provides a monthly stipend to caregivers who reside with and care for a MassHealth recipient age 16 and older. The member must have MassHealth Standard or Commonhealth and must have a permanent or chronic medical condition that requires daily hands-on assistance and/or cueing/supervision to successfully complete at least one personal care task. Family Caregiver Support Program

The Family Caregiver Support Program offers support to caregivers of any age caring for a loved one over the age of 60 as well as grandparents over the age of 55 caring for grandchildren under the age of 18 or an adult with a disability who is 18-59 years of age. The Caregiver Specialist, Karen Trilligan, can be a lifeline to community resources, information or emotional support. For more information, click the link below or contact a caregiving specialist.

Ombudsman

Every nursing home has an ombudsman available for any resident of any age who has concerns regarding their stay. While we encourage residents and families to first give the staff the opportunity to assist, there is a definite benefit to having an impartial person involved. If you need help identifying your ombudsman, or for questions about the service, contact Program Director Mary Brooks at 508-573-7235 or mbrooks@baypath.org.
One Care

One Care is a health care option for Massachusetts adults, age 21-64, who are eligible for both MassHealth and Medicare (dual eligible). BayPath provides assistance to One Care clients by providing Case Management in the Long Term Support Services Coordinator (LTSC) role. Eligibility for in-home agency services are among the options reviewed by the Case Manager. For more information, click the link below. To enroll in One Care, contact Mass Health at 1-800-841-2900. Nursing

The nursing department is available to screen and approve MassHealth members who are interested in attending Adult Day Health programs in the MetroWest area. 

To request a screening, call 508-573-7200 and ask for someone in the Nursing Department.
Personal Care Attendant

The Personal Care Attendant (PCA) Program allows MassHealth recipients of any age funding to hire their own personal care workers. The member must have MassHealth Standard or Commonhealth, have a permanent or chronic illness or disability and need daily, hands-on assistance with at least two personal care needs. The amount of funding is determined via clinical assessment by a BayPath nurse and occupational therapist. Options Counseling

Options Counseling assists individuals of any age who live with a disability connect with community resources, programs and services. Your Options Conselor will ensure that you understand your options and problem solve if obstacles arise so that you can make an informed choice on your your next steps. Options counseling is unbiased and provided at no cost. Suspicion of Abuse or Neglect

For children from birth through age 17, any one with concerns can contact the Department of Children and Families through their child-at-risk hotline at: 800-792-5200.

For concerns regarding the abuse of an individual with a disability who is age 18-60, anyone can contact the Disabled Persons Protection Commission at their hotline at:
