The Supportive Housing Program is a joint partnership between the MCDA and BayPath Elder Services. The goal of the program is to provide all residents living at 240 Main Street, 397 Bolton Street and 29 Pleasant Street with on-site services that support their independence, safety, enjoyment and well-being.
The following benefits are offered through the program:
On-Site, On-Call Personal Care Workers
The on-site staff members will be available from 7 a.m. to 11 p.m. to respond to the urgent issues of all residents. The on-site staff members are employed by Medical Resources Home Health Corp., who have partnered with BayPath and the MCDA. Medical Resources also provides on-call staff 24/7 to respond to emergency needs of residents. In addition to responding to urgent requests from residents, the staff will also provide services to current BayPath clients. Any resident of any of the three housing sites, regardless of age or if they receive BayPath services, can call the on-call staff for assistance with urgent issues.
As part of the program, there is a full-time Supportive Housing Program Coordinator who can provide information about programs and services and help residents access government benefits and community resources.
The coordinator is based at 240 Main Street and available for home or office visits at all three sites.
Educational and Social Activities
A number of educational opportunities are provided to residents, including healthy living programs, informational sessions and other topics of interest. We are also available on a limited basis with social activity programming in conjunction with the tenants' association.
Daily Meal Program
BayPath's Nutrition Department provides one congregate meal a day that is open to any and all residents of the three sites. Currently all meals are at 397 Bolton Street at 11:30 a.m. Monday-Friday.
