Chronic Disease Self-Management
(My Life, My Health)

Developed by Stanford University, this program is an overall lifestyle-management program for adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease and diabetes. The program provides information and motivation and teaches practical skills on managing chronic health problems. 

The goal is to enable participants to build self-confidence to assume an active role in maintaining their health while living with a chronic health condition. 

Meets for 6 weekly sessions, for two and one-half hours per session.
Chronic Pain Self-Management 

Developed by Stanford University and based on the principles and teachings of the Chronic Disease 
Self-Management Program, this program is for people living with a primary or secondary diagnosis 
of chronic pain such as: Fibromyalgia, Whiplash Injuries, Post-Surgical Pain, Neuropathic Pain, Neuralgias, Post-Stroke Pain, Diabetic Neuropathy, Muscular Pain/MS and other types of ongoing pain. 

Participants learn techniques to control pain, how to deal with anger and frustration, pacing activities, tips for rest and relaxation and how to evaluate new treatments.

Meets for 6 weekly sessions, for two and one-half hours per session.

Become An Active Self-Manager of Your Health!

These programs follow the guidelines of The Healthy Living Center of Excellence and are the result of years of research and development at Stanford University and top medical organizations that have proven positive results for participants. 

Workshops are held at senior centers, medical offices, libraries, fitness centers, places of worship or anywhere a group interested in health and wellness might gather together.

Programs are low or no-cost and are continually offered throughout MetroWest and beyond. The programs make use of volunteer leader talent to provide a peer-group model for participants. 
​Diabetes Self-Management

Developed by Stanford University and based on the Chronic Disease Self-Management curriculum, this program is for people with diabetes and their families and caregivers. 

Topics include: techniques to deal 
with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, appropriate exercise, medication usage and healthy eating. Participants make weekly action plans, share experiences and help each other solve problems they encounter in daily life. 

Meets for 6 weekly sessions, for two and one-half hours per session.

A Matter of Balance

This program provides practical coping strategies and simple balance and strength exercises to reduce the risk and fear of falling. Based on the view that falls and the fear of falling are controllable, this program encourages goal setting for increased activity, home and environment changes to reduce falls risk and shared group support to promote confidence. 

Meets for 8 weekly sessions, for two hours per session.
Tai Chi for Healthy Aging

Developed by Brookline Tai Chi with support from the Centers for Disease Control, the Massachusetts Department of Public Health and the Tufts Health Plan Foundation, Tai Chi for Healthy Aging brings the practice of Tai Chi to seniors with an emphasis on better balance for falls prevention. 

The 24-week program is designed for seniors to improve their strength and balance, reduce their fears and risk of falling, and increase their flexibility. 

Meets for 24 weekly sessions, for one-hour each session. 
Healthy Eating for Successful Living

This program is designed for older adults who want to learn more about nutrition and how lifestyle changes can promote better health. 

Based on the USDA MyPlate, the program focuses on nutrition education, goal setting, self-assessment of dietary patterns and group support. 

Meets for 6 weekly sessions, for two and one-half hours per session, followed by a restaurant outing.

Healthy Living Program Coordinator

Renee joined BayPath as the Healthy Living Program Coordinator in the spring of 2019. She comes to us with experience as a health policy advocate and served as a Resident Services Coordinator at RCAP Solutions, Inc. where she conducted out-reach, case management, implemented a comprehensive program of person-centered care coordination services, supporting 100+ low income seniors, families, and disabled individuals living in HUD subsidized housing. 

Renee has a B.S. in Physical Education/ Exercise Science from UMass Boston and a Health Management Certificate from the Graduate School of Public Health at UMass Lowell.

The Healthy Living Center of Excellence (HLCE) represents a unique collaboration of community-based organizations, aging service providers, health care systems, governmental agencies, and healthcare payors; all with the shared goal of transforming the traditional health care delivery system.

​The Center is founded on the belief that a collaborative partnership between activated older adults, their medical providers, and community- based services is the most effective model for managing the healthcare needs of an ever growing aging population.

Savvy Caregiver

The Savvy Caregiver Program is a free, six-session training series for family and friends who are active caregivers of a person with Alzheimer's or a related dementia.

Taking care of a person living with Alzheimer’s or related dementia is specialized work. 

This workshop will help you become an expert in caregiving for someone with Alzheimer’s or another form of  dementia by helping you understand the impact of dementia on both of you and then provide you with the skills you need to manage daily life.
